Community Services Case Management (CSCM)

One of the greatest challenges for families of individuals with moderate to severe disabilities is finding a program that can meet the developmental, physical, and behavioral needs of their loved one. The Arc offers community-based programs, resources, and referrals that provide a continuum of support. 

What's CSCM?

Community Services Case Management (CSCM) serves Bexar and surrounding counties by working with families who care for children and young adults ages 3-21 years with special health care needs and disabling conditions that Medicaid does not currently cover. Staff also  serves individuals of any age with cystic fibrosis. CSCM case managers help families navigate the network of service providers and advocate for the child’s best interest. The case management process is comprehensive and it involves everyone who plays a role in the child’s life: parents, guardians, family members, physicians, clinics, school personnel, childcare providers, and social service agencies. 

Case managers work with families to identify needs, create a personalized plan, and connect with resources. Most families served by CSCM qualify as low-income; however, The Arc’s case management services are provided free of charge to children with disabilities and their families regardless of income. 

Our Staff

At The Arc, we know that in order to provide excellent services, you have to have a great team. To that end, we constantly invest in staff training that will expand their toolkits to enable them to provide quality and innovative care to the individuals in our programs. We value a community of continuous learning and partner with subject matters experts who provide trainings on a variety of topics such as: basic medical supports, positive behavior supports, and emergency preparedness.

Eligibility

  • Diagnosis of an intellectual and/or developmental disability (IDD), autism spectrum, cerebral palsy, Down syndrome, dual diagnosis with IDD.
  • Individuals of any age with cystic fibrosis.
  • Ages 3-21 years.
  • Must reside in Bexar and surrounding counties.
